Locations
We operate 22 homes in the Twin Cities Metro Area located between HWY 169 and HWY 100 from St Louis Park to Brooklyn Park.
These homes include:
- Two 6-bed Licensed and Certified Intermediate Care Facility (IFC) Residential Homes
- Eight Licensed 4-bed High Medical Homes with 24 hour nursing services
- Twelve Licensed 4-bed Waiver Homes (some with Mid-Medical support including nursing services).
Placement in an HBI 245d licensed setting is funded through the following waivers: CADI, DD, CAC, BI.
Intermediate Care Facility (ICF) settings are funded through Medicaid.
