Traditional Waiver Homes
Our Traditional Waiver homes provide an empowering atmosphere where every person can pursue their dreams and live life to the fullest. In addition to 24-hour care provided by highly trained Direct Support Professionals, what truly sets Homeward Bound apart is our dedicated Activities Coordinator. This role focuses on creating engaging and enriching activities that promote social interaction, skill development, and overall well-being.
Mid Medical Homes
Our Mid-Medical homes boast a team of competent staff equipped to provide specialized care to individuals including those with G-Tubes, J-Tubes, or G/J Tubes. Additionally, staff are well-trained in managing specialized diets, seizures, repositioning, Passive Range of Motion exercises, EZ-Lift and other transfer needs. Staff at our Mid-Medical homes also conduct regular skin checks to ensure the safety and health of the people we serve.
High Medical Homes
Our High Medical Homes are fully equipped to support individuals with tracheostomies, mechanical ventilation, C-PAP and Bi-PAP therapies, deep suctioning, and heated humidity for enhanced comfort. Additionally, our trained Nurses are proficient in wound care, as well as the management of G-Tubes, J-Tubes, G/J-Tubes, and the administration of suprapubic and indwelling catheters, including intermittent catheterizations. This is not an exhaustive list, as we are committed to addressing a wide range of complex medical needs to provide personalized care for each individual.
Locations
We operate 22 homes in the Twin Cities Metro Area located between HWY 169 and HWY 100 from St Louis Park to Brooklyn Park. At Homeward Bound individuals have the opportunity to thrive as a member of their community living in a neighborhoods setting with active support and person-centered care 24/7/365. Placement in an HBI 245d licensed setting is funded through the following waivers: CADI, DD, CAC, BI. Intermediate Care Facility (ICF) settings are funded through Medicaid.
