Our Traditional Waiver homes provide an empowering atmosphere where every person can pursue their dreams and live life to the fullest. In addition to 24-hour care provided by highly trained Direct Support Professionals, what truly sets Homeward Bound apart is our dedicated Activities Coordinator. This role focuses on creating engaging and enriching activities that promote social interaction, skill development, and overall well-being.
Care is further enhanced through the use of an electronic health record system, which tracks medical needs with oversight from a Program Manager, Operations Administrator, and a Nurse Case Manager, ensuring that each individual receives coordinated and responsive care. Together, these elements foster a vibrant community where everyone can thrive.
About the Homes
Our Community Residential Settings are single-family, accessible homes located in residential neighborhoods throughout the northern and western suburbs of Hennepin County. Each of our homes serve four adults with disabilities matched based on their needs and preferences. Each individual enjoys a private bedroom personalized to their likes and interests.
