Community Alternative Care

Community Alternative Care (CAC) homes provide individuals access to 24-hour nursing care. These homes promote the health and independence of individuals in need of intensive health services who would otherwise require care in hospitals and nursing homes.

Our Homes

Our CAC locations are single-family, accessible homes located in residential neighborhoods in Maple Grove, Brooklyn Park, and Golden Valley. Each of our homes serve four adults with disabilities matched based on their needs and preferences. Each individual enjoys a private bedroom personalized to their likes and interests.

These homes provide high quality, trained, and caring staff and 24-hour on-site nursing. Extensive care and medical support are provided to help individuals maintain optimum health and quality of life. Individualized care and person-centered services are designed by the individual, their family, and the team in order to promote growth and independence at home and in the community.
