High Medical Homes
Homeward Bound’s High Medical homes beautifully blend the advantages of our Traditional Waiver and Mid-Medical homes with the addition of 24-hour in home support from licensed Nurses, delivering hospital-level care in the comfort of their own home. These homes are custom-built and equipped with advanced technology to ensure we meet the medical needs of our service recipients.
Our High Medical Homes are fully equipped to support individuals with tracheostomies, mechanical ventilation, C-PAP and Bi-PAP therapies, deep suctioning, and heated humidity for enhanced comfort. Additionally, our trained Nurses are proficient in wound care, as well as the management of G-Tubes, J-Tubes, G/J-Tubes, and the administration of suprapubic and indwelling catheters, including intermittent catheterizations. This is not an exhaustive list, as we are committed to addressing a wide range of complex medical needs to provide personalized care for each individual.
In these homes, our licensed Nurses and Direct Support Professionals work collaboratively to ensure that all the needs of the individuals we serve are met. This seamless teamwork fosters a comprehensive approach to care, combining hands-on support with medical expertise.
Direct Support Professionals in our High Medical Homes receive comprehensive training from skilled Nurses, who provide in-depth instruction on proper use of specialized medical equipment. This training includes hands-on experience with devices such as pummel machines, oxygen equipment, and suctioning machines, enabling Direct Support Professionals to assist Nurses effectively in addressing the complex needs of each person in the home. This rigorous training program ensures that our High Medical Direct Support Professionals are well-prepared and empowered to deliver the highest level of care in a supportive and responsive environment.
About the Homes
Our Community Residential Settings are single-family, accessible homes located in residential neighborhoods throughout the northern and western suburbs of Hennepin County. Each of our homes serve four adults with disabilities matched based on their needs and preferences. Each individual enjoys a private bedroom personalized to their likes and interests.
