Intermediate Care Facilities (ICF/ID) provide individuals with support, skill development, and personalized care in a licensed residential setting.
Our Homes
Our ICF/ID are accessible homes located in Maple Grove, Brooklyn Park, and Plymouth. Each of our homes serve six to eight children and/or adults with disabilities matched based on their needs and preferences.
Our homes provide high quality, trained, and caring staff and 24-hour nurse consultation services. Our Brooklyn Park homes also have nursing on-site 24 hours a day and provide Services During the Day for individuals with high medical needs who are unable to attend an off-site program. Individualized care and person-centered services are designed by the individual, their family, and the team in order to promote growth and independence at home and in the community.
