Mid Medical Homes

In the realm of supportive housing, Homeward Bound’s Mid-Medical Homes offer all of the benefits of our Traditional Waiver homes with a significant advancement in medical care.

Our Mid-Medical homes boast a team of competent staff equipped to provide specialized care to individuals including those with G-Tubes, J-Tubes, or G/J Tubes. Additionally, staff are well-trained in managing specialized diets, seizures, repositioning, Passive Range of Motion exercises, EZ-Lift and other transfer needs.  Staff at our Mid-Medical homes also conduct regular skin checks to ensure the safety and health of the people we serve.

Care in our Mid-Medical Homes is significantly improved through the implementation of an advanced electronic health record system. This system meticulously tracks each individual’s medical needs, all under the direct supervision of a Program Manager, Operations Administrator, and a Homeward Bound Nurse Case Manager.

Our innovative Mid-Medical nursing model ensures excellent support with a high nurse-to-service recipient ratio, averaging one Nurse Case Manager for every three Mid-Medical Homes. This approach allows us to deliver personalized and attentive care, tailored to the unique needs of each person we serve.

About the Homes

Our Community Residential Settings are single-family, accessible homes located in residential neighborhoods throughout the northern and western suburbs of Hennepin County. Each of our homes serve four adults with disabilities matched based on their needs and preferences. Each individual enjoys a private bedroom personalized to their likes and interests.
